This is a young adults church event. We ask everyone to honor the culture of the house, follow leadership guidance, and help create a safe and respectful environment for everyone attending.

I, the undersigned participant (“Participant”), give my permission and consent to travel, attend, and participate in all scheduled activities during the Summer Camp of Iglesia Internacional de Las Vegas dba Grace City Church (“Grace City Church”). I agree to abide by all Grace City Church rules and policies established by its Directors and leadership.

I acknowledge and fully understand that participation in this event involves inherent risks, including but not limited to accidents, injuries, illness, transportation incidents, and other unforeseen circumstances. I voluntarily accept and assume all risks associated with my participation.

On behalf of myself, my heirs, assigns, and legal representatives, I hereby waive, release, and discharge Grace City Church, Iglesia Internacional de Las Vegas, its pastors, staff, volunteers, church board members, other participants, and any car, truck, or bus drivers or other persons involved with the event, from any and all liability, claims, demands, actions, or causes of action of any kind arising out of or related to my participation in this event.

I further authorize Grace City Church staff or volunteers to obtain or authorize emergency medical care or treatment that may be reasonably necessary as a result of an accident, injury, or illness occurring before, during, or after the Event if I am unable to provide consent myself.

I understand and acknowledge that Grace City Church does not provide medical or health insurance coverage, nor accident insurance for participants. I accept full financial responsibility for any medical treatment and hereby release Grace City Church, Iglesia Internacional de Las Vegas, its pastors, staff, and volunteers from any monetary liability.

Este es un evento de jóvenes adultos de la iglesia. Pedimos a todos honrar la cultura de la casa, seguir la dirección del liderazgo y ayudar a crear un ambiente seguro y respetuoso para todos los asistentes.

Yo, el participante abajo firmante (“Participante”), doy mi permiso y consentimiento para viajar, asistir y participar en todas las actividades programadas durante el Campamento de Verano de Iglesia Internacional de Las Vegas, conocida como Grace City Church (“Grace City Church”). Acepto cumplir con todas las reglas y políticas de Grace City Church establecidas por sus Directores y liderazgo.

Reconozco y entiendo plenamente que la participación en este evento implica riesgos inherentes, incluidos, entre otros, accidentes, lesiones, enfermedades, incidentes de transporte y otras circunstancias imprevistas. Voluntariamente acepto y asumo todos los riesgos asociados con mi participación.

En nombre propio, de mis herederos, cesionarios y representantes legales, por la presente renuncio, libero y eximo de responsabilidad a Grace City Church, Iglesia Internacional de Las Vegas, sus pastores, personal, voluntarios, miembros de la junta directiva, otros participantes y cualquier conductor de automóvil, camioneta o autobús, o cualquier otra persona involucrada en el evento, de toda responsabilidad, reclamo, demanda, acción o causa de acción de cualquier tipo que surja de o esté relacionada con mi participación en este evento.

Asimismo, autorizo al personal o voluntarios de Grace City Church a obtener o autorizar cualquier atención o tratamiento médico de emergencia que razonablemente sea necesario como resultado de un accidente, lesión o enfermedad que ocurra antes, durante o después del Evento si no puedo dar mi consentimiento por mí mismo.

Entiendo y reconozco que Grace City Church no proporciona seguro médico ni seguro contra accidentes para los participantes. Acepto la responsabilidad financiera total por cualquier tratamiento médico y por la presente libero a Grace City Church, Iglesia Internacional de Las Vegas, sus pastores, personal y voluntarios de cualquier responsabilidad económica.
