Grace City Youth we are going to camp! Under the stars and among the desert mountains, we will spend 4 days and 3 nights surrounded by the nature of His creation.  The entire GC Youth Leadership Team is super excited and filled with expectation for what God is going to do in your hearts of during our Summer Camp 2025. We know that it will be a time where you will be instructed in the Word of God, encouraged through friendships and challenged to participate in EXTREME GAMES. In order to get you on a team and begin all the camp prep, please answer the following questions. 

Grace City Youth nos vamos a campamento! Bajo las estrellas y entre las montañas desérticas, pasaremos 4 días y 3 noches rodeados por la naturaleza de Su creación.  Todo el Equipo de líderes de GC Youth estamos súper emocionados y con grandes expectativas por lo que Dios va a hacer en tu corazón durante nuestro Summer Camp 2025.  Sabemos que será un tiempo donde ustedes serán instruidos en la Palabra de Dios, animados a través de tiempo de compañerismo y retados a participar en JUEGOS EXTREMOS.  Para poder organizar equipos e iniciar los preparativos para campamento se requiere que contesten las siguientes preguntas.

Waiver / Consentimiento

I, as the guardian of the “registrant” give permission/consent for my son/daughter who is a minor, to travel, attend and participate in all scheduled activities during the Summer Camp of Iglesia Internacional de Las Vegas dba Grace City Church. I accept and require that my son/daughter abide by all Grace City Church rules established by the Directors.
I fully accept and understand the risks of my minor son/daughter participating. On behalf of myself and the minor, I waive all legal action and release volunteers, Pastors, other teens, Church board members, car, truck, or bus drivers, or anyone else from any liability or consequences related to Grace City Church, from all claims or obligations of any kind arising from participation in this event.
In addition, I consent and empower volunteers to give their consent to authorize any medical care or treatment that may seem reasonably necessary for the minor, as a result of an emergency, accident or illness if it occurs before, during or after the Event. I understand that Grace City Church does not provide health insurance or insurance in the event of an accident and I surrender my rights and release all monetary liability to Grace City Church/International Church of Las Vegas, Pastors and volunteers.

Yo, el/la responsable o guardián del “registrado”, le doy permiso a mi hijo/hija que es menor de edad, para viajar, asistir y participar en todas las actividades durante la Actividad Programada de Iglesia Internacional de Las Vegas dba Grace City Church. Yo acepto y requiero que mi hijo/hija obedezca todas las reglas de C3 Church establecidas por los Directores.
Acepto y entiendo los riesgos de que mi hijo/hija menor participe. A nombre mío y del menor, renuncio a toda acción legal y libero de toda responsabilidad o consecuencia a los voluntarios, Pastores, otros adolescentes, miembros de la mesa directiva de la Iglesia, pilotos de los autos, camionetas, o autobús, o cualquier persona relacionada con Grace City Church, de todas las reclamaciones u obligaciones de cualquier tipo que se deriven de la participación de este evento.
Además, autorizo y faculto a los voluntarios a dar su consentimiento para autorizar cualquier atención o tratamiento medico que pueda parecer razonablemente necesario para el menor, como consecuencia de una emergencia, accidente o enfermedad si se llegase a producir antes, durante o después del Evento. Entiendo que Grace City Church no provee seguro de salud, ni seguro en caso de un accidente y renuncio de toda responsabilidad monetaria a Grace City Church, Pastores y voluntarios.
